A Guide to Managing an Overactive Bladder

A Guide to Managing an Overactive Bladder
Taikhum Sadiq

Overactive Bladder or OAB is an incontinence condition that is characterized by a sudden and frequent urge to urinate and usually occurs during the night as well as the day. When an individual has to pass urine frequently throughout the day without being able to control it, they are usually diagnosed with OAB. Another important symptom of an overactive bladder is the sudden discharge of urine, also known as urgency incontinence.

What are the Symptoms of an Overactive Bladder?

There are four major symptoms of OAB, although the intensity of the symptoms and their time of occurrence might differ from person to person

  • Sudden urge to urinate that is incontrollable
  • Urgency incontinence or the sudden discharge of urine involuntarily
  • Frequent urination, up to 8 times a day
  • Multiple trips to urinate during the night also referred to as nocturia

Causes of Overactive Bladder or OAB

There are a few direct causes of an overactive bladder and they include:

  • Neurological disorders
  • Urinary tract infections
  • Hormonal changes in women during menopause
  • Diabetes
  • Enlarged prostate or prostate cancer
  • Tumors or bladder stones

There are a few risk factors that might indirectly cause an overactive bladder and they are:

  • Side effects of certain medications
  • Excess consumption of caffeine or alcohol
  • Incomplete bladder emptying
  • Decline in cognitive functioning
  • Stress, especially excessive stress due to work

How to Treat an Overactive Bladder?

There are multiple ways to treat an overactive bladder but none of them guarantee a complete resolution of the condition. A wide range of treatment methods can help reduce the intensity of the symptoms and help an individual lead a comfortable and secure life. Treatment options include

Pelvic Floor Muscle Exercises

Also known as kegel exercises, pelvic floor muscle exercises are one of the most common and effective ways of strengthening the muscles of the bladder that control the outflow of urine. The strength of these muscles determines the amount of control one can have over their urge to urinate. People affective with an overactive bladder often tend to have weakened pelvic floor muscles and so kegel exercises can help strengthen them to reduce the effects of OAB. Kegel exercisers are one of the most common mediums of helping a person increase the output of kegel exercises.

Other important ways to help treat an overactive bladder include making some important changes in your lifestyle. These factors directly or indirectly affect the health of the excretory system and hence can reduce the effects of OAB.

Check your weight – Obesity is an important factor in the occurrence of an overactive bladder. Always ensure that the BMI of an individual is within the stipulated margins so as to reduce the chances of OAB.

Bladder training – Bladder training includes training the muscles of the bladder to control the flow of urine. This translates to delay urinating when you feel the urge to do so. But this method is only recommended to individual who face very subtle bouts of OAB. Those who suffer from severe OAB are recommended not to use this technique.

Diet – Excessive fluids throughout the day can increase the urine produced in the body and can in turn reduce the strength of the pelvic muscles by filling the bladder continuously. Also try to avoid excessive consumption of caffeine and alcohol.

Managing Overactive Bladder

There are two major ways of managing an overactive bladder. They are:

Intermittent Catheters

Intermittent catheters are recommended for individuals who do not require constant catheterization for urine discharge. Intermittent catheters are designed to be comfortably and safely inserted into the urethra whenever an individual requires emptying of the bladder. The intermittent catheter helps easy drainage of urine into a drainage bag so that they user is not forced to get up and go to the washroom each time. They are mostly recommended for people who are bedridden, use a wheelchair, cannot walk to the bathroom, or the elderly.

External Condom Catheters

External condom catheters are designed for men who suffer from incontinence and overactive bladder. They are external sheaths that can be worn over the penis whenever an individual wishes to empty their bladder. The condom catheter is connected via tubing to a drainage bag that collects the urine for easy disposal. They are reusable and can be easily stored and used in outdoor settings as well.

Adult Briefs, Underpads, and Adult Underwear

Incontinence products such as adult briefs, underpads, and adult underwear are one of the most common ways of managing an overactive bladder. They can be easily worn under your daily wear and can help avoid embarrassing situations by helping absorb urine. They are recommended for people who also suffer from urgency incontinence along with OAB.

Wash the glans penis from the urethral opening (tip) to the base of the glans with soap, water and washcloth or unscented disposable wipe. Wash with a circular motion from the tip outward. Do not wash back and forth over the urethral opening. Repeat the washing two more times with different areas of the wash cloth or with a new unscented wipe each time.
  • While holding the penis upright with one hand at a 45 to 60-degree angle from the body, slowly insert the lubricated catheter into the penis using your dominant hand. Gently slide the catheter until you meet resistance at the urethral sphincters. Do not pull the catheter in and out when you meet resistance; just apply gentle but firm continuous pressure until the catheter advances into the bladder.
  • Continue to insert the catheter until urine begins to flow. If you are standing in front of the toilet or sitting on it with legs spread apart, the urine can drain into the toilet. If you are lying down, place the funnel end of the catheter into a container to collect the urine. Hold it in place until urine stops flowing.
  • When urine stops flowing, slowly rotate the catheter between your fingers while withdrawing the catheter. If urine starts to flow again when withdrawing the catheter, stop each time it flows and let the urine drain. • Always wash your hands before beginning. Trim hairs on the shaft and base of your penis, and any other pubic hair that might interfere with the adhesive on the catheter.
  • In order for the catheter adhesive to perform properly, the skin must be clean and very dry. Wash your penis with mild soap and warm water. Be sure to allow the area to dry completely before proceeding.
  • Place the rolled self-adhering catheter over the penile tip. Slowly and carefully unroll the catheter, leaving just enough space in the funnel end so the tip of your penis does not rub against the inside of the funnel. If uncircumcised, be sure to leave the foreskin in place over the head of the penis.
  • Smooth out any wrinkles in the catheter. Gently squeeze the catheter to properly seal it to the skin. Wear time may be significantly reduced if adhesive is not properly sealed.

Connect to Leg Bag

Strap the collection bag to your thigh or calf, depending on your activities and your clothing. Connect one end of the tubing to the catheter and cut the tubing to a comfortable length. Connect the cut end to the leg bag. Always empty your leg bag before it becomes uncomfortably full and pulls on the catheter.
